PARENT RELEASE

RELEASE: I undersigned, as a parent and/or guardian of a minor, ask that he be admitted to participate in the Kick on 4th Down camp. In consideration of such admission, I do hereby agree to release Kick on 4th Down and its employees of and from all causes, liabilities, damages, claims, or demands whatsoever on account of any injury or accident involving the said minor arising out of the minor’s attendance at the practice or in the course of competition and/or activities held in connection with the camp.

High School Coach Invite:

Kick on 4th down would like to invite all coaches to attend this camp free of charge as long as they have a player who is registered for the camp. I encourage the coaches to come and listen \ watch what we are teaching your players, so you can coach them during the season.

What to wear

What to bring

- 2 Footballs (name or school clearly marked)
- Kicking Tees
- Kicking Holder

*Please bring shoes other then cleats for when we go into the classroom*
